Hadfield protects patient information.

We maintain physical, electronic, and organizational safeguards to protect patient information. We continually review our practices and policies, and monitor our computer networks to help us ensure the safety of patient information.

We may record that you have read and approved our Notice of Privacy Policy. The pharmacy will obtain written authorization before using or disclosing PHI for purposes other than those provided or otherwise permitted by law.

• We may communicate with persons involved in your care.
• We may contact you about refilling your medications on a schedule.
• We may contact you to remind you to refill medications or to present health care alternatives or announcements of services.
• We may disclose to the FDA any adverse reactions that we are aware of or any information requested in product recalls.
• We may disclose information to comply with workers compensation laws.
• We may disclose information to insurance companies or other entities performing audits.
• We may disclose information in response to a court order, subpoena, or other administrative order.
• We may disclose information about you to researchers if an institutional review board has approved the research protocols.
• We may disclose information about you to coroners and funeral directors.
